Have you tried:
1 - Routing something OTHER than Insert Sends to these ADAT outputs (Main
Outs, Auxes etc)?
2 - Switching the EDS card / MEC assignment in the Master Mixer (ie, if the
troublesome ADAT outs are on MEC1 connected to Submix 1 playing off CardA,
does the same problem exist when you assign MEC1 / CardA to Submix 2 etc)
3 - Changing your EDS cards (basically, label them, pull them out, and put
them in the same PCI slots, in a different order - put a different EDS card
in the slot for the ADAT MEC).
I had a similar problem, but it was ADAT OUT 3 and 4 on MEC1 connected to
EDS Card A in a three card system on WinME. On this same MEC, there were a
total of (2) ADAT, (2) 8-input, (2) 8-output.
I had the same symptoms though - swapped ADAT cards and OUT 3 and 4 still
didn't wor, but the "bad" card suddenly worked in a different hole. The
"good" card displayed the same problem as the "bad" one.
I yanked and moved the EDS cards in my system and connected the MEC to a
different Card A and the problem went away with both the original ADAT and
the swapped ADAT.
I concluded it to be an EDS Insert problem on a specific EDS card - not an
OS thing. Something about the way the Insert Sends were handled on that EDS
card. Could be a loose connection on a board, a bad or dying chip etc. I
never bothered to check if you could route Auxes or Mains to the same
questionable outputs (and if I couldn't, it would be a bigger problem).
